Survodutide
Weight Loss, Metabolic Health
In short
- A dual glucagon and GLP-1 receptor agonist in development by Boehringer Ingelheim and Zealand Pharma.
- Investigational, not approved anywhere.
- Phase 2 reported roughly 19% mean weight reduction at 46 weeks.
- Has shown notable results in MASH (metabolic dysfunction-associated steatohepatitis), where the glucagon arm drives hepatic fat oxidation.
- Unlike retatrutide it omits GIP agonism — dual rather than triple.
Structure
How it works
Survodutide activates the glucagon and GLP-1 receptors. GLP-1 agonism reduces appetite and improves glycaemic control; glucagon receptor agonism increases energy expenditure and promotes hepatic fat oxidation.
The glucagon component is the reason for the liver-directed development programme — hepatic fat reduction is a more direct consequence of glucagon signalling than of GLP-1.
What the research shows
Phase 2 in obesity reported approximately 19% mean weight reduction at 46 weeks at the highest dose.
A phase 2 trial in MASH reported significant improvement in liver histology versus placebo, which is a harder endpoint than weight and is the more clinically notable result.
Phase 3 programmes in obesity and MASH are ongoing.
Dosing in the literature
Figures below are quoted from published studies, with the species or population they were measured in. They are records of what was studied, not recommendations.
| Studied in | Amount | Route | Source |
|---|---|---|---|
| Human — phase 2 obesity trial, titrated over 46 weeks | Escalated to 3.6 or 6.0 mg weekly | subcutaneous | PubMed |
| Human — phase 2 MASH trial | Escalated to 2.4, 4.8, or 6.0 mg weekly | subcutaneous | PubMed |
Commonly reported ranges
These are amounts people report using. They are not from any study, have not been evaluated for safety, and are reproduced here only because they circulate widely and are better seen alongside the evidence than instead of it.
| Reported by | Amount | Route |
|---|---|---|
| Commonly reported, mirroring trial titration | 1–6 mg weekly, escalated slowly | subcutaneous |
No published human pharmacokinetic study establishes a dose, interval, or duration for this compound. Community figures below have no trial behind them and are reports of practice, not evidence. No approved product exists, so grey-market material has unverified identity, purity, and concentration.
Handling and storage
Supplied as a lyophilised powder requiring reconstitution with bacteriostatic water. The powder is comparatively stable; once reconstituted it is refrigerated and used within weeks. Track the date you reconstitute each vial — the Inventory section does this, and it is the difference between knowing a vial is three weeks old and guessing.
Weekly subcutaneous injection with slow escalation, following the trial pattern.
Side effects
Nausea, vomiting, diarrhoea, and constipation dominate the side-effect profile across this entire class. They are dose-related, worst after each escalation, and are the reason titration schedules exist. Serious but uncommon events include pancreatitis and gallbladder disease, the latter more likely with rapid weight loss. Delayed gastric emptying is now addressed in pre-operative anaesthesia guidance because of aspiration risk from retained stomach contents.
Glucagon receptor agonism adds dose-dependent heart rate increases, reported across the glucagon-containing agents.
Weight regain after discontinuation is consistent across this class and is not a failure of the drug — appetite regulation returns to baseline when the receptor stops being activated. Any decision to start should account for what stopping looks like.
As an unapproved compound, what is actually in a grey-market vial is an additional unquantifiable risk.
Combinations
Combining with other incretin agonists duplicates GLP-1 agonism and compounds gastrointestinal effects.
With insulin or sulfonylureas, hypoglycaemia risk rises and dose reduction of those agents is typically required.
Research status
Phase 2 results in obesity and MASH have been published, with phase 3 ongoing. Legitimate pharmaceutical development by Boehringer Ingelheim and Zealand Pharma.
Legal status
Not approved in any jurisdiction. Investigational, with phase 3 ongoing in both obesity and MASH. Sale for human consumption is unlawful in most countries, and grey-market material is manufactured entirely outside any regulated supply chain.
WADA status
Not specifically named on the Prohibited List. Falls under WADA class S0, which prohibits any pharmacological substance not currently approved by any governmental regulatory health authority for human therapeutic use. Treat it as prohibited in sport.
Conclusion
Survodutide sits between tirzepatide and retatrutide in both mechanism and reported effect. Its MASH histology data is arguably the more significant finding, because liver histology is a much harder endpoint than weight.
As with retatrutide, the science is real and the consumer supply chain is not.
